I am able to take insurance as well as private pay if you wish to see me as an out-of-network provider unless you have Medicare or Medicaid, in which case I cannot accept cash payment and you will need to see a provider who is in-network with Medicare or Medicaid.

Out-of-network plans - If you have plans other than those listed above, I will bill your insurance and you may receive reimbursement, depending on your plan. You would pay the full fee at each session.

I will want to know the following before scheduling:
Your insurance
If you are looking to schedule for therapy, medication evaluation, or both
If you are currently taking psychiatric medication
Your diagnoses from past providers, if known
If you are transferring care from another psychiatric provider
If I am not taking new patients, I will offer referrals
Some information that may help you decide if we are a good fit:
Please be aware that I will not continue prescribing regular, scheduled use of benzodiazepines such as Xanax or Klonipin. If you are taking these medications regularly, a part of your treatment will be tapering (decreasing) and stopping these.
I treat adults only (21 and over) in my practice.
I do require a $100 deposit to schedule an intake, and this is to discourage people from making an intake appointment and not showing up. It will be used for future deductibles and/or copayments, or refunded if you don’t owe copayments or deductible. It is not returned or refunded if you do not come to the appointment (you may reschedule up to 48 hours prior to the appointment). If this is a hardship for you, please let me know and we can make other arrangements.
Prior to your first appointment, I will have you fill out and sign some paperwork. You can either print this and bring it to the appointment, or I will give you instructions on uploading it to my scheduling and payment site.
Please note that I have online scheduling, which I will make available once we have discussed the above and decided to schedule an intake.
